Texas health cOMMUNITY IMPACT GRANT -
2021/2022

Program objectives to reach by December 2022

  • Decrease depression and/or anxiety among low-income individuals age 40+.
  • Increase food security and navigation services among low-income individuals age 40+.

vIsion. Leadership. Teamwork

Texas Health Community Impact makes creative and resourceful use of cross-sector collaboration grants to serve local community needs. This data-driven, results-oriented approach identifies the North Texas ZIP codes that are most in need of assistance and directs how we work with leaders, influencers, and existing resources in those underserved communities. Together, we're addressing health inequities, addressing the core causes of chronic disease, and giving tools to promote long-term health and wellness.
